 What is the Integrated Seniors Team? This is a partnership program of three agencies – Punjabi Community Health Services (PCHS), Supportive Housing of Peel (SHIP) and Peel Seniors Link (PSL). This program provides services to seniors in their homes who at risk but with intervention can remain at home living independently. This service will be provided in North West Brampton with a focus on promoting seniors’ health and wellness. The Integrated Seniors Team provides risk management, assessment and education, personal support services and access to supportive housing options  Who can benefit? Seniors 65 years of age and over.  What are the major benefits? The benefits are substantial and include: Services are in English, Punjabi, Hindi, and Urdu The team Case Managers and Personal Support Workers (PSW) connects seniors with the appropriate services during the time of crisis. Homemaking, Personal Support/Attendant Services, ongoing friendly visits and light home maintenance are also provided to the seniors based on their needs.  How are referrals made? Referrals are made by calling 905.677.0889. Community Based Agencies, Religious leaders, Family Members, Family Physicians and individuals may make a referral.  Where is the Program located? The program is located at 2829 Derry Road East, Mississauga, Ontario, L4T 1A5   What are the hours of operation? Monday to Friday from 9:00 a.m. to 5:00 p.m. with extended hours based on clients’ availability. |
