HART Hub Brampton Scheduled to Open January 2026

HART Hub Brampton is scheduled to open in January 2026.

The HART Hub Brampton is a new community-based hub offering integrated mental health, addictions, and housing-related supports for individuals in Brampton and surrounding areas.

HART Hub Brampton is being delivered in collaboration with eight service delivery partners and in partnership with the Region of Peel. The hub will be located at 10 Peel Centre Drive, allowing clients to access Region of Peel services in the same location, including housing supports, income assistance, and other social services.

The HART Hubs operate using a no wrong door approach, meaning individuals can access support without an appointment, and staff will help connect them to appropriate services based on their needs. Services are free and designed to be welcoming, accessible, and client-centred.

HART Hub Brampton: Roles of Service Delivery Partners

HART Hub Brampton is delivered by a group of direct service agencies working together as an integrated team. Each organization contributes staff and expertise to support individuals accessing the hub, while services are coordinated through a shared, collaborative model of care.

  • Canadian Mental Health Association (CMHA) Peel Dufferin: Supports coordination across partner agencies, contributes mental health and addictions expertise, and oversees hub operations to ensure services are delivered in a coordinated, community-based manner.
  • Indus Community Services: Contributes culturally responsive, trauma-informed, person-centred support to ensure services are accessible and inclusive, with a vocational counsellor providing employment-focused guidance, skills development, and pathway planning to support recovery, stability, and community integration.
  • Moyo Health and Community Services: Provides health promotion, education, and support services for individuals living with, affected by, or at risk of HIV.
  • Peel Addiction Assessment and Referral Centre (PAARC): Provides assessment, navigation, and referral support related to substance use, behavioural dependencies, addictions and mental health concerns.
  • Punjabi Community Health Services (PCHS): Provides culturally and linguistically responsive peer support, counselling, navigation, and transitional case management to help individuals and families feel safe, supported, and connected to care.
  • Roots Community Services: Provides culturally relevant supports for Black, African, and Caribbean communities, contributing to services that are culturally affirming and responsive.
  • Services and Housing In the Province (SHIP): Supports access to housing-related services and connections to community-based housing supports, promoting stability and continuity of care.
  • WellFort Community Health Services: Provides primary care and health-related supports as part of the integrated hub model, contributing to coordinated care for individuals with complex needs.

Through this collaborative approach, partners support individuals accessing the HART Hub and connect them to services that best meet their needs.
